What is a Home Medicines Review?

A Home Medicines Review (HMR), also known as a Medication Management Review (MMR), is a Medicare-funded service designed to help people living at home better understand and manage their medications.

This service is conducted by a Credentialed Pharmacist in collaboration with a patient’s GP. If you are taking multiple medications, recently had a hospital stay, or are experiencing side effects, an HMR can help ensure your treatment is safe, effective, and easy to follow.

How It Works

  1. Your GP Refers You
    Your doctor completes a referral to Whiteway Clinical via secure email.

  2. I Contact You
    I will get in touch to arrange a convenient time to visit your home.

  3. We Review Your Medications Together
    I spend time reviewing your prescriptions, supplements, and over-the-counter medicines to ensure you understand each one and are using them safely.

  4. I Provide a Report to Your Doctor
    I send a comprehensive report with recommendations to your GP, who will review them with you at your next visit.

Who Is Eligible?

  • You may be eligible for an HMR if:
  • You are living at home (not in residential care)
  • You take multiple medications or have complex health needs
  • Your GP believes you would benefit from a review
  • Only your doctor can initiate an HMR, so speak with them to discuss your eligibility.
