What is an RMMR?

A Residential Medication Management Review (RMMR) is a collaborative service provided by a Credentialed Pharmacist to assess and optimise the medication regimen of residents in aged care facilities. It is designed to improve patient safety, reduce medication-related harm, and support better health outcomes.

This Medicare-funded service is delivered in partnership with a resident’s GP and the aged care facility’s care team.

How It Works

  1. Agreement Established
    A Service Agreement is created between Whiteway Clinical and the residential care facility.

  2. GP Referrals Submitted
    Referrals are received from residents’ GPs to initiate the review process.

  3. In-Person Review Conducted
    I visit the facility, meet with staff if needed, and review the resident’s medications.

  4. Clinical Report Provided
    A detailed report with medication review outcomes and clinical recommendations is provided to the referring GP and facility team.
